Care Services
Can care plans change over time?
No two people remain exactly the same over time. Someone recovering from surgery may need less assistance after several weeks. A client living with dementia may gradually require additional support. A person who initially needed help only twice a week may later benefit from daily visits.
These changes are completely normal. For this reason, every MyCG care plan is designed to grow and change alongside the individual.
Rather than creating a care plan once and leaving it unchanged, we continually evaluate whether it still reflects the client's current needs and goals.

1. Why Care Plans Change
There are many reasons why a care plan may need to be updated. Common examples include:
Recovery after illness or surgery.
Changes in mobility.
Increased independence.
New medical diagnoses.
Memory changes.
Hospital discharge.
Medication changes.
Family availability.
Safety concerns.
Client preferences.
Lifestyle changes.
Every change provides an opportunity to ensure care remains appropriate.
2. Who Can Request a Care Plan Review?
Care plan reviews are not limited to MyCG staff. A review may be requested by:
The client.
Family members.
A legal representative.
The Care Coordinator.
The assigned caregiver.
A physician or healthcare professional (where appropriate).
Another authorized member of the care team.
We encourage families to speak with us whenever they believe adjustments may be helpful.
3. How Does MyCG Know When Changes Are Needed?
Our caregivers spend meaningful time with clients and often notice gradual changes that might otherwise go unnoticed. For example, they may observe:
Increased difficulty walking.
Reduced appetite.
Changes in mood.
New confusion.
Increased fatigue.
Difficulty completing daily activities.
Changes in sleep patterns.
Safety concerns around the home.
These observations do not result in independent medical decisions. Instead, they help identify when a conversation or formal care plan review may be appropriate.
4. Regular Communication Is Essential
Care planning is a collaborative process. When changes are being considered, MyCG works closely with:
The client.
Family members.
Care Coordinators.
Healthcare providers when appropriate.
Other authorized care team members.
Open communication helps ensure everyone understands what has changed, why changes are recommended, how care will be adjusted, and what the family can expect moving forward.
5. Care May Increase—or Decrease
Many people assume care plans only become more intensive. That is not always true. As clients recover or regain independence, care may actually decrease. For example, a client recovering from hip surgery may initially require daily visits. Several weeks later, they may only need assistance twice a week. Our goal is always to provide the right amount of care—never more and never less than what is appropriate.
6. Every Change Is Personalized
Adjusting a care plan does not mean starting over. Often, small changes make a significant difference. Examples include:
Increasing visit frequency.
Reducing visit frequency.
Adding meal preparation.
Adding transportation.
Introducing companion care.
Adjusting personal care routines.
Modifying household assistance.
Updating safety precautions.
Changing preferred visit times.
The care plan continues evolving alongside the client.
7. The Role of CareOS
CareOS plays an essential role in ensuring care plans remain accurate and up to date. The platform securely maintains:
Current care plans.
Care goals.
Scheduled services.
Caregiver observations.
Supervisor reviews.
Family requests.
Care plan revisions.
Approval history.
Effective dates for changes.
Version history.
Every authorized caregiver always sees the most current approved care plan, helping ensure consistent care across every visit.
8. Care Planning Is a Continuous Journey
At MyCG, we do not think of care planning as a one-time event. We see it as an ongoing conversation. As life changes, care changes. As needs evolve, support evolves. This flexible approach allows clients to receive care that continues to match their lives—not simply their past circumstances.
9. Who This Article Is For
This article is especially helpful for:
Families beginning home care.
Current MyCG clients.
Adult children supporting aging parents.
Clients recovering after illness.
Referral partners.
10. Common Misunderstandings
"The care plan never changes after services begin." Care plans are designed to evolve as the client's needs change.
"Only MyCG can decide to change a care plan." Clients and families are encouraged to request reviews whenever they believe adjustments may be beneficial.
"Changing the care plan means something is wrong." Not at all. Many care plan updates reflect positive progress, recovery, or changing goals.
"Every change requires creating an entirely new plan." Most updates involve refining or expanding the existing care plan while preserving continuity.
11. When Should You Contact MyCG?
Please contact us if your loved one experiences changes such as:
Increased falls.
Hospitalization.
Changes in mobility.
Memory concerns.
Appetite changes.
Increased caregiver stress.
Recovery after illness.
Changes in family availability.
New personal preferences.
The earlier we discuss these changes, the better we can adapt the care plan to meet current needs.
12. Why Families Appreciate Flexible Care Planning
Families often tell us one of the things they appreciate most is knowing that MyCG's care does not remain fixed while life changes around it. They value having a care team that listens, observes, communicates, and adapts. Rather than expecting clients to fit into a predetermined schedule, we adjust our services to reflect each person's evolving journey. That flexibility helps families feel confident they are receiving the right care at the right time.
