How Can We Report a Concern or Complaint?

At MyCG, we welcome honest communication. Whether your concern is small or significant, we want to know about it. Sometimes a simple conversation is all that is needed. Sometimes a situation requires investigation and corrective action. Either way, every concern deserves attention.

1. What Should You Tell Us About?
You do not have to wait for a major problem before contacting us. We encourage families to contact MyCG whenever they have questions, suggestions, concerns, compliments, requests for clarification, changes in care needs, safety concerns, scheduling concerns, or communication concerns.
Early conversations often prevent small issues from becoming larger ones.
2. Examples of Concerns Families Might Raise
Families commonly contact us about a change in a loved one's condition, scheduling issues, caregiver punctuality, communication concerns, questions about the care plan, billing questions, requests for additional services, caregiver compatibility, safety observations, or documentation questions.
Every concern matters.
3. How Can You Contact MyCG?
We want reporting concerns to be easy. Families may contact us through telephone, email, the MyCG Family Portal, the MyCG Family App, their Care Coordinator, during scheduled care reviews, or any other approved communication channel.
Choose whichever method feels most comfortable.
4. Step 1 — We Listen
First, we listen carefully. We want to understand what happened, when it happened, who was involved, and what outcome you are hoping for. We do not make assumptions.
5. Step 2 — We Document
Your concern is securely documented. This helps ensure nothing is forgotten, appropriate follow-up occurs, trends can be identified over time, and accountability is maintained.
6. Step 3 — We Review
Depending on the situation, MyCG may speak with caregivers, review visit documentation, review CareOS records, consult supervisors, review care plans, or gather additional information.
Every concern is reviewed fairly and objectively.
7. Step 4 — We Respond
Once we understand the situation, we discuss what we found, any actions being taken, any care plan adjustments, and next steps.
Our goal is open communication throughout the process.
8. Step 5 — We Learn
Whenever appropriate, we use the experience to improve training, policies, communication, care processes, and supervision.
Continuous improvement benefits every family we serve.
9. What If My Concern Is Urgent?
Some situations require immediate attention, such as client safety concerns, suspected abuse or neglect, medical emergencies, serious caregiver misconduct, or threats to health or wellbeing. If your loved one is experiencing a medical emergency, call 911 or your local emergency services immediately. For urgent care concerns that are not life-threatening, contact MyCG as soon as possible so we can respond quickly.
10. Will My Concern Be Taken Seriously?
Absolutely. Every concern is treated respectfully. We understand that speaking up can sometimes feel uncomfortable. Our commitment is to create an environment where families feel safe sharing honest feedback. Your voice helps us improve.
11. What About Compliments?
We also love hearing what is going well. Families often tell us about exceptional caregivers, positive experiences, acts of kindness, outstanding communication, and excellent service. We share this feedback with our caregivers because recognition encourages continued excellence.
12. The Role of CareOS
CareOS helps ensure concerns are managed consistently and transparently. Every concern can be securely tracked from beginning to resolution. CareOS will support concern logging, complaint categorization, priority levels, investigation workflow, assigned responsibility, follow-up reminders, resolution documentation, family communication history, trend analysis, and executive quality reporting.
This ensures that no concern is overlooked.
