What Happens During the In-Home Assessment?

Every home is different. Every family is different. Every person is different.

That is why MyCG never creates care plans based only on a telephone conversation. The in-home assessment allows us to see the environment where care will actually be provided and understand the small details that often make the biggest difference.

Our goal is not to evaluate your home. Our goal is to understand how your loved one lives so we can provide care that feels natural, comfortable, and safe.

1. Why Is an In-Home Assessment Important?

A telephone conversation tells us what support may be needed. An in-home assessment helps us understand how that support should be delivered. It allows us to:

Get to know your loved one personally.

Observe daily routines.

Identify safety opportunities.

Understand family involvement.

Develop an individualized care plan.

Match the right caregiver.

Build trust before care begins.

This visit forms the foundation for everything that follows.

2. Understanding Your Loved One

We take time to learn about personal history, daily routine, health concerns, mobility, communication preferences, hobbies and interests, cultural preferences, religious practices, and personal goals.

We believe understanding the individual is just as important as understanding the care needs.

3. Understanding Daily Life

We may discuss morning routines, meal preparation, medication routines, bathing and dressing, household activities, exercise, social activities, and sleep patterns.

Understanding normal routines helps caregivers provide support in a respectful and familiar way.

4. Home Safety Review

The assessment also includes a general review of the home environment. This is not a formal home inspection. Instead, we look for opportunities to improve safety and comfort. Examples may include fall risks, lighting, stairways, entryways, bathroom accessibility, mobility pathways, and emergency access.

If we notice something that may improve safety, we will discuss it with you respectfully.

5. Understanding Family Involvement

Families play an important role in successful home care. We discuss who currently provides support, how family members would like to stay involved, preferred communication methods, emergency contacts, scheduling preferences, and family goals.

Our role is to complement your family's support—not replace it.

6. Care Preferences

We also discuss personal preferences that help caregivers provide more individualized care, such as favorite meals, preferred routines, music, television, pets, visitors, cultural traditions, and communication style.

Sometimes these details become the most meaningful part of the care relationship.

7. Questions You Can Ask

The assessment is also your opportunity to ask anything you'd like. Families often ask about caregiver matching, scheduling, communication, pricing, family updates, care documentation, safety, emergencies, CareOS, and future flexibility.

8. What Happens After the Assessment?

Following the visit, MyCG prepares a personalized care plan, service recommendations, caregiver matching recommendations, a personalized quotation, scheduling options, and next-step guidance.

Everything is reviewed with your family before care begins.

9. Do We Need to Prepare?

Not really. The most important thing is simply being yourselves. However, you may find it helpful to have family members present if possible, think about your goals, prepare questions, gather any information you feel is relevant, and consider your preferred schedule.

There is no need to make your home "perfect." We are there to understand your loved one's everyday environment—not to judge it.

10. What Makes the MyCG Assessment Different?

Many organizations focus primarily on tasks. MyCG focuses first on people. We don't simply ask, "What services do you need?" We ask, "How does your loved one live?" That difference changes everything. Instead of designing care around a checklist, we design care around a person's life.

"The assessment is a medical examination." No. It is a conversation and observational visit focused on understanding care needs.

"Someone is coming to inspect our home." No. We simply look for ways to provide safer, more effective care within your existing environment.

"We need to clean or prepare the house perfectly." Not at all. We want to understand everyday life—not a staged presentation.

"We need to decide everything during the visit." No. The assessment is one step in a collaborative planning process. Your family will have time to review recommendations before making decisions.

13. When Should You Schedule an Assessment?

The sooner planning begins, the more options families usually have. Many families schedule assessments before hospital discharge, when care needs begin changing, following a fall, when family caregivers need additional support, or as part of future planning.

Planning ahead often reduces stress later.
